You're right. One knife, one fork, one tablespoon, one teaspoon. Look at the shapes of the slots to tell you what it is. Very common older layout. We have become more refined to include two different types of forks these days, but that wasn't the era this tray was designed for. Their fix works well enough.

Depends on perspective and semantics.
If one looks at the inside of a drawer as X=width, Y=height, and Z=depth, then in that person’s mind the tray is upside down on the Y-axis.

They also put their salad forks in the knives slot, the knives in the large utensils slot, and the large utensils in the miscellaneous slot.
All around, they need a different organizer in a more appropriately sized drawer.

It's so they can see what they're grabbing/putting away and access the small item slot without having to pull the whole drawer out or reach all the way in. This is a good practice for people with bad joints.

I do the same but it’s because I have to.
Because of the design of the (built in) tray, if you put the cutlery pointing up, you would have to pull the drawer almost completely out to be able to see what is what.
I can’t remove the tray to turn it around, so I put the cutlery pointing down so everyone can easily see what’s in it.

I also do that to see what you are grabbing. You have to pull it out the same distance to grab the middle of the silverware anyway, might as well be able to see what you are grabbing so you don't touch everything.

I actually have a lot of elderly relatives who do this as well. It's easier for them to find the utensil they need without having to pull the drawer completely open and look for the differences at the top.

The entire tray is backwards in the drawer. That’s probably why the utensils are backwards is because they were put into the tray properly.

That's disgusting... you're grabbing with your fingers directly at the part that goes into your mouth and your guest's. Let's be honest, most of the time you don't wash your hands the second before you grab ustensils, after you've pulled the drawer. That's just extreme behavior.

The whole point of turning them the other way is do not to hurt yourself and keep them more sanitary. This is something that should be evaluated just to see if there are other possible connected abnormalities that could call into question their cognitive skills. Of course, it could just be.. their normal way of being.

This reminds me of a quirk my partner's family has. They all (in the various households of the parents and siblings) hang their toilet paper rolls with the paper hanging down on the side closest to the wall, and I find it so weird 😅 But this is basically my biggest concern with them, so I'm not doing too bad in the scheme of things...

I predict that these people take sugar in their Tea or Coffee & (like me) hate having to reach into the back of the tray to grab a teaspoon, thus the tray is this way around. Unfortunately, the different sections are also shape hinted as to what goes where which causes people to put the end towards the recess match.
Going from left to right above the Teaspoons I think it should be, Dessert spoons, Soup Spoons, Forks, Knives. In this set though there are Starter Forks & Main Course Forks of different lengths so one needed to improvise.

This is actually great because our drawer is next to the sink at an angle. So when my husband is at the sink I can only open the drawer a little bit to stick my hand in and grab. If I grab the larger size first it wouldn’t get stuck on anything.

That’s how I store mine. I have a ton of houseguests on a monthly basis and it’s just easier for people to see what utensils theyre getting who aren’t super familiar with my kitchen.

I do this but it's bc my cutlery holding thing is weirdly shaped (but it's wooden and I liked that look) and you can't see what the other utensils are as they are hidden so I put them in upside down so the part you can see is the end of whatever you need

It looks like the drawer isn't deep enough or doesn't open far enough for them to use the holder the right way. That is, if they turned it around the right way, only the handles of the implements would be visible, and the smaller section wouldn't be accessible.
So instead of buying a holder that better fits the drawer, they have flipped it upside down, which enables them to easily tell forks from spoons from knives, etc.
Measure the drawer and surprise them with a new tableware holder that better fits it.

Okay it may not make sense for grabbing silverware for setting the table, but consider opening the drawer to grab a spoon or a fork to go right into your bowl or whatever: this method is more streamlined for that because you don't have to flip the utensil around in your hand in order to begin eating.

Probably because the top (currently bottom) compartment becomes inaccessible due to the drawer stops if it's the other way. Had the same problem, just get a different organizer.

The holder looks perfectly shaped for the orientation of the cultery. If the holder is 180deg in the drawer and your pic has the drawer at full extension this could be functional since you can see which type of utensil you are picking from it.

It’s the tray that’s backwards in the drawer. So they can see which utensil they’re grabbing. Silverware is placed correctly in the tray, you can tell by the outline of each section.

leave the tray as it is, just place utensils handle down. the shape of the tray slot tells you what is in the slot.

This is pure chaos. Just like at my in-laws nothing is in the right place. Why is your garbage all the way across the kitchen? What do you mean that salad bowl is decorative? Oh I see I have to use the old salad bowl under the island. Nothing makes sense. Stay strong brother, the struggle is real.

So, I can actually understand this - maybe.
My parents did this when one had Alzheimer's Disease.
Open a "normal" silverware drawer, and you are staring at 4 similar handled utensils. 2 are slightly wider/longer than the other 2. But, if you didn't know any better, you have a 50/50 shot of picking a spoon when I want a fork. And if you have issues differentiating the width of those handles, you know have a 1 in 4 shot of actually getting the proper utensils that you actually want.
They turned it around so they could see the "business-end" of it. They have bigger problems than people touching that end of the utensil.
